There are many reasons to visit a podiatrist, from toenail problems to foot, leg and heel pain. That being said, it’s always best to consult a podiatrist rather than letting the problem get out of hand and wishing you had come sooner!

At Delray Beach Podiatry and Boynton Beach Podiatry, Dr. Ian S. Goldbaum, a podiatric physician and surgeon with over 30 years of experience, has all of the right tools and knowledge needed to help get you back on your feet.

As it is with most other doctors, however, your first visit to our offices will typically consist of several background questions and some paperwork that will serve to not only help Dr. Goldbaum understand your problems, but also help him get to know you a little better.

To facilitate the process, you can print out new patient forms online, and for everything else, please check out our information guide for all newcomers below!


  • Make a list of any symptoms and questions you have.
  • Make a list of all medications and any previous surgeries.
  • Gather and bring important medical records and laboratory test reports from other doctors or hospitals.
  • Call to make sure that your insurance is accepted at our offices.
  • Call to inform our office if you have special needs.


  • Insurance card(s)
  • Driver’s license or other picture ID
  • New patient forms (signed)
  • List of prescription & over-the-counter medications
  • Relevant medical records


  • Go over your list of questions.
  • If you do not understand what Dr. Goldbaum is saying, be sure to ask for further explanation.
  • Listen carefully to everything Dr. Goldbaum has to say during your appointment.
  • Discuss your symptoms and any recent changes in your health may have noticed. This is often more important than you think!
  • Talk about all new medications you are currently taking and will be taking.
  • Ask why these new medications have been prescribed, and how to take them.
  • Inform Dr. Goldbaum of any allergies you may have.
  • Let Dr. Goldbaum know if you are being treated by other doctors.


  • Prepare for any tests Dr. Goldbaum orders based on the results of your visit.
  • Ask about what you need to do to get ready, possible side effects, and when you can expect results.
  • Ask when and how the test results will be made available to you.
  • Remember to schedule a follow-up appointment (if necessary) at the front desk before you leave the office!
